                                    Date Night

                                    Parents, are you looking for an evening alone and can't find a babysitter? Leave your kids at the museum, that way the adults and the children will enjoy their evening out!

                                                             Please read all of the detailed information below.




                                    Ages Accepted
                                     All children aged 3 years to 11 years of age (must be toilet trained) are invited to participate regardless of race, religion, or special need. Children with special health care needs will be served if reasonable accommodations can be made for their participation in the program.  
                                      
                                    Sign In and Out
                                    Parents/Guardians will be required to sign their child/children in and out of the program during each shift.

                                    Payment
                                    Fees are
                                    to be paid in advance of services. Parents/Guardians will be required to pay a minimum donation (see below for rates) per each 3 hour shift (whether or not your child attends the entire shift).  Cash, check, or credit card will be accepted. No refunds will be given. Rates are as follows:

                                    Regular Date Night= $20 per non-member child; $15 per
                                    member child

                                    Downtown Discovery Night*= $65 per non-member family; $50
                                    per member family (prices are for families with up to 2 children- each additional child $10)

                                    *Downtown Discovery Nights require at least 5 family participants. Event will be canceled if minimum registrants not
                                    met.


                                    Behavior
                                    Children will be expected to respect others and the toys
                                    and equipment at the CWCM. Redirection and gentle reminders will be used to guide behaviors. For any more serious behavior issue, parents/guardians will be contacted.  CWCM reserves the right to deny services to any child with repeated serious behavior issues.

                                    Items Parents/Guardians Need to
                                    Supply
                                    *Sippy Cup, labeled with child’s first and last name (if
                                    child is not able to drink from regular cup)

                                     *Comfort Items such as a blanket or stuffed toy, if
                                    necessary (CWCM is not responsible for lost or stolen
                                    toys)

                                     *A complete change of clothing is recommended for toddlers and preschoolers

                                    Sick Child
                                    If a child should exhibit any of the following
                                    symptoms, parents/guardians (or if not available, we will contact others listed
                                    on the child enrollment form) will be contacted and asked to pick-up their child
                                    immediately:
                                    *vomiting
                                    *diarrhea
                                    *fever
                                    *unusual rash
                                    *child not well enough to participate in activities

                                    A sick child will be isolated until parents/guardians or other authorized persons are able to pick up the child.  It is extremely important that parents/guardians list a cell phone number or persons who would be immediately available if an emergency arises.



                                    Medication
                                    Medication will not be administered during the Downtown
                                    Date Night program.

                                    Injuries
                                    In the event of a minor injury (scratches, bruises or bumps), volunteers would wash the area with soap and water and apply ice and/or bandages. In the event of a medical emergency, volunteers would call 911 and then parents/guardians immediately.

                                    Light Snack
                                    A light snack of crackers and juice will be provided. Children should come to the museum having already eaten supper.

                                    Caregivers
                                    Volunteers and paid staff will be caring for the children. We will strive for a 1:4 ratio of volunteers to children.

                                    CWCM Staff Caregivers:
                                    -CPR Certified
                                    -Have all passed Portage County background checks
                                    -On-site at all times
                                    -Early Childhood educational experience


                                     Registration

                                     Parents/Guardians are required to pre-register for the Date Night program by stopping by the museum and filling out the required Enrollment, Health History, and Parent/Guardian Consent Forms. Registration may be made over the phone with a credit card if the parent has email access and can fill out the required forms electronically.
                                     
                                    Authorized Persons for Pick Up
                                    Children will only be released to those persons listed on
                                    the child enrollment form (with no exceptions).  We reserve the right to ask for a photo identification to protect the safety of the children. Please pick up your child(ren) by 9pm. There will be a $1 per minute fee assessed for every minute past 9pm that the child(ren) are not picked up. If you are more than 15 minutes
                                    late picking up your child(ren) you will not be allowed to participate in the Date Night program again.

                                    The following forms need to be completed before leaving your children with CWCM's child care service. Completed forms can be mailed to 1100 Main Street attention Abby Bergeron or emailed to abby@cwchildrensmuseum.org
